眼球内容除去後に発生した悪性黒色腫の1症例

Development of malignant melanoma 40 years after evisccration

緒　言

　眼球内容除去をうけた眼に，後年，悪性黒色腫が発生したという報告は数例ある^1〜7）。これらの症例においては，悪性黒色腫が，残存ブドウ膜より発生するのか，あるいは，眼球内容除去の原疾患の病因が，実は悪性黒色腫であり，残存腫瘍より再発したものか，などの議論がなされてきた。またこのことから，眼球内容除去術の適応となる全眼球炎や，絶対緑内障の原因として，悪性黒色腫の存在に，注意が喚起されてきた本編では，40年前に眼球内容除去術をうけ，義限台を装用していた眼に，悪性黒色腫を発生した興味ある症例を経験したので記載し，このような症例についての問題点を考按してみたい。

A 59-year-old female developed exophthalmus and pain in her left eye, in which evisceration of the globe and implantation of a prosthesis had been performed 40 years before for unidentifiable disease. We performed exenteration of the orbit and detected malignant melanoma with diffuse extraocular extension. The malignant melanoma was of mixed type and seemed to have arisen from the residual uveal tissue. The presence of the prosthesis may have activated the tumor by irritation. An enuclea-lion is usually to be preferred to evisceration because of such complications.
