後部強膜炎による再発性視神経乳頭炎

Recurrent optic papillitis in a case of posterior scleritis

　両眼に後部強膜炎による再発性視神経乳頭炎を呈した18歳女性症例を報告した。症例は，右眼の眼痛，毛様充血，視力低下にて発症し，前房内炎症，中心視野欠損と視神経乳頭の発赤腫脹を認め，視神経乳頭炎の像を呈した。MRIで後部強膜の肥厚とこれに接する眼窩組織の高信号輝度を認めたことから後部強膜炎による視神経乳頭炎と診断した。ステロイド剤の全身投与によく反応したが，その約3か月後に両眼に，さらに約3か月後に左眼に視神経乳頭炎を再発し，いずれもステロイド剤全身投与で軽快した。

An 18-year-old female presented with ocular pain, ciliary injection and visual loss in her right eye. The affected eye showed inflammation in the anterior chamber, central scotoma and hyperemia of the disc. Magnetic resonance imaging (MRI) showed thickening of the posterior sclera and high intensity in the adjacent orbital tissue. These find-ings led to the diagnosis of papillitis secondary to posterior scleritis. The condition improved after systemic corticosteroid and recurred bilaterally after 3 months and again in the left eye another 3 months later. Systemic corticosteroid was effective at each episode.
