後部強膜炎の臨床経過

Clinical features of posterior scleritis

緒　言

　強膜炎の名のもとに，臨床上広く親しまれてきた疾患は，強膜前半部に主病変の場がある，いわゆる前部強膜炎に属する型である。

　これに対して，後部強膜炎は後極部附近におきる強膜の炎症病変の総称で，1902年Fuchs^1）が，視力障害，球結膜の充血と浮腫，眼痛および乳頭，網膜の硝子体内への膨隆を認めた1例を，後部強膜炎として報告したのを初めとするが，今日までこの疾患の臨床例は極めて少なく，かつ，ほとんどが1例報告にすぎない。著者らはFuchs^1）の診断基準に合致する例を1972年に経験して以来，現在までに3症例を経験したので，その臨床所見，経過について報告する。

This paper describes the clinical picture and the course of posterior scleritis in 3 patients. One was a male aged 29 years and the other two were fe-males aged 12 and 25 years. The condition started with unilateral conjunctival hyperemia at the equator of the globe, ocular pain, pain during eye movement and visual disturbance. Funduscopy showed hyperemia of optic disc, peripapillary retinal edema and manifest macular wrinkles. Fluorescein angiography showed dye staining of the disc with numerous fluorescein dots in the papillomacular region in one case. Dye staining of the disc and deeper choroidal layers was seen in another.
