眼窩腫瘍が疑われた眼窩血栓性静脈炎の臨床病理所見

Clinicopathological manifestations in a case of orbital thrombophlebitis simulating orbital tumor

　68歳男性に発症した，眼窩血栓性静脈炎の臨床所見と病理所見を報告した。左側の眼球突出と眼球運動制限があり，CTで左眼窩内側に軟部組織様の異常陰影がみられた。臨床所見から左眼窩腫瘍を疑い，摘出した。病理組織学的には，異常に拡張した静脈腔の中に器質化しつつある血栓が存在し，さらに静脈壁に炎症細胞浸潤がみられた。本症例のように，強い炎症を伴う眼窩静脈血栓症の病理報告は稀である。

A 68-year-old male presented with proptosis and limitation of ocular movement in his left eye.Computerized tomography (CT) showed a mass of soft tissue adjacent to the nasal wall of the left orbit. The mass was surgically extirpated under the tentative diagnosis of orbital tumor. Histopath-ology showed organized thrombus in abnormally dilated venous channels and inflammatory cells in venous walls. The case proved to be a rare instance of orbital thrombophlebitis with severe inflamma-tion.
