パルス療法後の再発性後部強膜炎の1例

A case of recurrent posterior scleritis treated by pulse systemic corticosteroid

　60歳女性が左眼瞼腫脹と眼痛で受診した。過去5年間，2〜3か月ごとに強膜炎が反復していた。左眼矯正視力は0.6であった。強膜充血，虹彩炎，胞状網膜剥離，黄斑部雛襞形成があり，超音波検査で眼球後壁の肥厚があった。後部強膜炎と診断した。デキサメタゾン4mg／日の内服で寛解したが，3か月後に再発した。2日間のステロイドのパルス療法ののち，プレドニゾロン30mg／日の内服とベタメタゾンの眼球周囲注射で寛解した。2年2か月後に後部強膜炎が再発したが，プレドニゾロン3mg／日の内服とベタメタゾンの眼球周囲注射で寛解が得られた。

A 60-year-old female presented with lid swelling and pain in her left eye. She had had recurrent scleritisevery 2 or 3 months during the past 5 years. Her left eye showed hyperemia, iritis, bullous retinal detachment and foldin the macula. Ultrasonography showed thickening of the posterior sclera. She was diagnosed with posterior scleritis. Thefindings improved following peroral dexamethasone 4 mg/day. Scleritis relapsed 3 months later. Cure was obtained aftertwo days of pulse corticosteroid therapy followed by peroral prednisolone and periorbital injections of betamethasone.Another relapse 26 months later subsided after peroral prednisolone and periorbital injections of betamethasone.
