Abstract Purpose:To report a case of dacryocystocele that extended posteriorly and developed impaired vision and eye position.

Case:A 61-year-old female presented with changed outlook in her right face since 2 months before. She had repeated acute episodes of right dacryocystitis since the third decade of life. Each episode was treated by antibiotics and massage.

Findings and Clinical Course:Corrected visual acuity was 0.6 right and 1.2 left. She showed swollen right lacrimal cyst and upward deviation with proptosis of the right eye. The right eye showed choroidal folds and dilated choroidal vessels in the posterior fundus. A cystic tumor was present nasal and inferior to the right eyeball by computed tomography(CT)and magnetic resonance imaging(MRI). Dacryocystorhinostomy was performed under the diagnosis of dacryocystocele. Malignancy was ruled out pathologically in the excised specimen. The eye position improved soon after surgery with visual acuity of 1.0. The nasolacrimal passage showed patent function.

Conclusion:This case illustrates that dacryocystocele may invade the retrobulbar space in the orbit. Dacryocystorhinostomy was beneficial in the present case.
