Abstract Purpose:To compare the intermediate-term outcome of primary open-angle glaucoma(POAG)and exfoliation glaucoma(XFG)cases treated with EX-PRESS® Glaucoma Filtration Device(Alcon)implantation.

Methods:This study involved 32 eyes of 26 cases that underwent EX-PRESS® implantation at Osaka Medical College. Between the POAG group(18 eyes, 14 cases)with the XFG group(12 eyes, 14 cases), we compared and analyzed the surgical success rate, intraocular pressure(IOP)reduction, medication and visual field score, endothelial density, and the numbers of scleral flap suture and laser-suture lysis during a followed-up period of more than 3-years postoperative. Surgical success was defined as(A)IOP<21 mmHg and(B)IOP<15 mmHg.

Results:In the POAG group and XFG group, the survival rate at 1- and 3-years postoperative was 96% and 95%, and 78% and 60%, respectively, for A, and 92% and 83%, and 62% and 33%, respectively, for B. A significant difference in surgical success B was found between the two groups(p=0.01). The XFG group had significant high IOP at 3-and 6-months and 1-and 3-years postoperative and a significant medication score at 3-years postoperative. In all other parameters, no significant difference was found between the two groups.

Conclusion:Our findings indicate the possibility that the intermediate-term outcome of EX-PRESS® implantation for XFG is poorer compared with that for POAG.
