Abstract Purpose:To report a case who developed corneal epithelial lesion following systemic treatment with TS-1®.

Case:A 64-year-old female was referred to us for epiphora, pain and impaired visual acuity in the right eye lasting for 3 years. She had received mastectomy for right breast cancer 18 years before. She had started systemic medication for metastases to the pelvis 8 years before. Chemotherapy with TS-1® was started 7 years before.

Findings and Clinical Course:Corrected visual acuity was 0.6 right and 1.2 left. Both eyes showed vortex-like irregularity in the corneal epithelium in both eyes. Cessation of treatment with TS-1® was followed by crack-like epithelial irregularity in both cornea one month later and improved visual acuity to 1.2 in either eye. Re-introduction of TS-1® was followed by progression of epithelial lesion to the corneal apex with decreased visual acuity. Withdrawal of TS-1® was followed by improved corneal lesion and visual acuity.

Conclusion:Systemic treatment with TS-1® seems to have induced suppression of upper corneal stem cells and led to irregularity of corneal epithelium. Careful follow-up is needed regarding irreversible changes in corneal stem cells.
