巨細胞性動脈炎による虚血性視神経症の1例

Ischemic optic neuropathy in a case of giant cell arteritis

要約　目的：巨細胞性動脈炎に併発した虚血性視神経症の1例の報告。

症例：85歳女性が2か月前からの右眼視力低下で受診した。同時期に咀嚼時の顎関節痛があった。

所見と経過：矯正視力は右手動弁，左0.2であった。両眼の視神経乳頭が蒼白で，乳頭陥凹はなかった。血液検査で白血球増加，CRPと血沈が亢進し，全身の炎症が推定された。これらの所見から巨細胞性動脈炎による虚血性視神経症が疑われ，ステロイドパルス療法を行った。浅側頭動脈の生検で，巨細胞動脈炎に矛盾しない所見が得られた。初診から87日後に他界するまで，左眼視力は0.3を維持した。

結論：進行した巨細胞性動脈炎によると推定される虚血性視神経症に対するステロイドパルス療法で，片側の視力が維持された。

Abstract　Purpose：To report ischemic optic neuropathy in a case of giant cell arteritis.

Case：An 85-year-old female presented with impaired vision in the right eye since two months before. She had noted pain in the jaws while chewing at about the same period.

Findings and Clinical Course：Best-corrected visual acuity was hand motion right and 0.2 left. Both eyes showed pale optic disc without cupping. Blood studies showed elevated sedimentation rate. She was treated by one session of pulsed steroid therapy. Biopsy of superficial temporal artery showed findings of giant cell arteritis. Her visual acuity remained at 0.3 left until decease 87 days after her initial visit.

Conclusion：Pulsed steroid therapy was effective in maintaining vision in one eye in the present case of ischemic optic neuropathy probably secondary to giant cell arteritis.
