Abstract Purpose:To report a case of optic neuritis that developed following cure of herpes zoster ophthalmicus.

Case:A 75-year-old female was referred to us for impaired vision in her right eye. She had been treated with tocilizumab for rheumatic arthritis.

Findings and Clinical Course:Corrected visual acuity was 0.5 right and 1.0 left. She showed pseudodendritic ulcer in the right cornea and vesicles in the area supplied by the first branch of right trigeminal nerve. She was diagnosed with herpes zoster ophthalmicus and tocilizumab was discontinued. She was treated by topical and systemic acyclovir and visual acuity in the right eye improved to 1.2 three weeks after her initial visit. Four weeks after the complete recovery, visual acuity in the right eye abruptly decreased to light perception. Ophthalmological examinations showed normal findings. Magnetic resonance imaging(MRI)showed findings suggestive of right optic neuritis. Her right visual acuity spontaneously improved to 0.7 three months later.

Conclusion:Optic neuritis in this case appears to be parainfectious optic neuritis and to be evoked by the sudden withdrawal of tocilizumab.
