Abstract. Purpose:To report endophthalmitis due to Paecilomyces lilacinus in an eye implanted with sulcus-fixated intraocular lens(IOL). Case:A 79-year-old female. The iris and IOL in the right eye luxated into the subconjunctival space 6 weeks after IOL implantation, apparently due to domestic violence. Visual acuity improved to 0.6 after repositioning of the iris and IOL sulcus fixation. The right eye developed endophthalmitis with hypopyon 2 months later. Vitreous specimen showed Paecilomyces lilacinus. Systemic 3 types of antifungal agents and intraocular injection of amphotericin B and micafungin were performed. Final visual acuity was light perception after repeated exacerbations. A reversible mecular edema in the left eye was observed during the antifungal treatment. The isolated fungus was resistant to amphotericin B and sensitive to micafungin and voriconazole. Conclusion:This case illustrates that endophthalomits due to Paecilomyces lilacinus may result in poor outcome, necessitating prevention and early treatment.
