眼内レンズ縫着術後に生じた外傷性Paecilomyces lilacinus眼内炎の1例

Trauma-induced endophthalmitis due to Paecilomyces lilacinus in an eye with sulcus-fixated intraocular lens

要約　目的：鈍性外傷後に行われた眼内レンズ縫着眼に生じたPaecilomyces lilacinusによる眼内炎の報告。症例：79歳女性。右眼に白内障囊外摘出と眼内レンズ挿入が行われ，その6週後に右眼虹彩と眼内レンズが結膜下に脱出した。原因は家庭内暴力と推定された。虹彩の整復と眼内レンズ縫着が行われ，視力は0.6に回復した。その2か月後に前房蓄膿を伴う眼内炎が発症した。硝子体による眼内洗浄を行い，Paecilomyces lilacinusが同定された。3種類の抗真菌薬の全身投与と眼内注射が行われた。再燃を繰り返し，最終視力は光覚弁になった。投薬中，左眼に一過性黄斑浮腫を認めた。薬剤感受性試験で分離された真菌には，当初用いられたアムホテリシンBには感受性が低く，ミカファンギンとポリコナゾールに感受性が高かった。結論：Paecilomyces lilacinusによる眼内炎は予後が不良であり，感染の予防と早期治療が重要である。

Abstract.　Purpose：To report endophthalmitis due to Paecilomyces lilacinus in an eye implanted with sulcus-fixated intraocular lens（IOL）. Case：A 79-year-old female. The iris and IOL in the right eye luxated into the subconjunctival space 6 weeks after IOL implantation, apparently due to domestic violence. Visual acuity improved to 0.6 after repositioning of the iris and IOL sulcus fixation. The right eye developed endophthalmitis with hypopyon 2 months later. Vitreous specimen showed Paecilomyces lilacinus. Systemic 3 types of antifungal agents and intraocular injection of amphotericin B and micafungin were performed. Final visual acuity was light perception after repeated exacerbations. A reversible mecular edema in the left eye was observed during the antifungal treatment. The isolated fungus was resistant to amphotericin B and sensitive to micafungin and voriconazole. Conclusion：This case illustrates that endophthalomits due to Paecilomyces lilacinus may result in poor outcome, necessitating prevention and early treatment.
