要約　目的：亜急性連合性脊髄変性症とWernicke脳症に併発した欠乏性視神経症の症例の報告。症例：67歳男性が2週間前からの両眼視力障害で受診した。10年前に舌癌で放射線治療，9年前に胃癌で胃切除を受け，1年前からふらつきがあり，最近は極端な小食と偏食をしていた。所見：矯正視力は右0.08，左0.09で，中心暗点があった。血中の総ホモシステインが高くビタミンB₁₂不足が考えられ，亜急性連合性脊髄変性症の疑いでビタミンB₁₂の投与を受けた。初診から2週間後に意識障害が生じ，磁気共鳴画像検査（MRI）でWernicke脳症と診断され，ビタミンB₁とB₁₂の点滴で意識を回復した。1か月後に視力が左右とも0.5になり，初診時に記録不能であった視覚誘発電位が記録可能になった。結論：視神経症の診断では，欠乏性視神経症の可能性を考慮し，ビタミンB群の検査が重要である。視機能の経過観察に視覚誘発電位が有用であった。

Abstract.　Purpose：To report deficiency optic neuropathy in a case of subacute combined degeneration and Wernicke encephalopathy. Case：A 67-year-old male presented with impaired vision in both eyes. He had received radiotherapy for lingual cancer and resection of stomach for gastric cancer before. He had felt staggering since one year before. He was in poorly nourished state due to loss of appetite. Findings：Corrected visual acuity was 0.08 right and 0.09 left. Both eyes showed central scotoma. Hematological studies showed vitamin B₁₂ deficiency suggesting subacute combined degeneration. He lost consciousness 2 weeks later. Magnetic resonance imaging led to the diagnosis of Wernicke encephalopathy. Treatment with vitamin B complex resulted in visual acuity of 0.5 in either eye one month later with recovery of visual evoked potential. Conclusion：Nutritional deficiency may be causatively involved in optic neuropathy.
