急性涙囊炎を契機に発見された若年性の涙囊部悪性リンパ腫の1例

A case of juvenile lacrimal sac malignant lymphoma with acute dacryocystitis as the initial manifestation

要約　目的：涙囊炎症状を呈した若年の涙囊部悪性リンパ腫の症例の報告。

症例：29歳男性。左涙囊部腫脹のため近医を受診し，左涙囊炎の診断で治療を開始され，いったん症状は改善した。数か月後に再発したが，治療により改善がみられないため精査加療目的で当科を紹介された。

所見：左涙囊部に発赤と腫脹があり，左眼球は上外側に偏位し，画像検索で眼窩から副鼻腔にかけて腫瘤性病変を認めた。生検での病理所見より悪性リンパ腫と診断され，化学療法を行った。治療後，涙囊部腫脹は改善し寛解が得られた。

結論：涙囊部腫瘍は涙囊炎を併発することがあるため，治療に抵抗性のある場合は若年であっても腫瘤性病変を念頭に置き，早期の画像検索を行う必要がある。

Abstract　Purpose：To report a case of juvenile lacrimal sac malignant lymphoma following symptoms of acute dacryocystitis.

Case：A 29-year-old male was referred to us for recurring of swelling of the left lacrimal sac that had started 3 months before.

Findings and Clinical Course：The area of left lacrimal sac was swollen. The left eye showed deviation in the upper lateral direction. A tumor mass was detected in the orbit by imaging. Biopsy led to the diagnosis of malignant lymphoma. Chemotherapy was followed by regression of the tumor.

Conclusion：The present case illustrates that lacrimal sac tumor may simulate acute dacryocystitis even in a young adult.
