角膜輪部デルモイド手術例の視力予後

Visual prognosis after lamellar keratoplasty for limbal dermoid cyst

　表層角膜移植を行った角膜輪部デルモイド13例13眼につき，視力予後に影響する因子，特に屈折に関して検討した。13例中6例（46％）に2D以上の遠視性不同視が存在した。術後の最高視力と乱視度との関係では，最高視力0.8以上の9眼の乱視度は平均2.8D,0.7以下の4眼の平均は3.8Dで，有意の差はなかった。術後の最高視力と患眼と健眼の屈折力の差との関係では，最高視力0.8以上の9眼の患眼と健眼の屈折力の差は平均1.6D,0.7以下の4眼の平均は4.1Dで，有意の差を認めた。視力予後に関して，大きな斜乱視とともに，遠視性不同視の存在が重要と思われた。手術による乱視の減少効果は平均0.5Dと少なく，手術は美容目的となるので，弱視治療後に行うのが望ましいと考えられた。

We evaluated the visual prognosis and the state of refraction in 13 eyes with limbal dermoid cyst treated by excision and lamellar keratoplasty. Hypermetropic anisometropia greater than 2.0D was present and averaged 4.3D in 6 eyes in theseries. The corrected visual acuity was better than 0.8 in 9 eyes and less than 0.7 in 4 eyes. There was no significant difference in the amount of astigma-tism between the two groups.

The difference in the power of refraction between the affected and the healthy fellow eye averaged 1.6D in the group with good visual acuity and 4.1D in the group with fair visual acuity. The difference was significant (p<0.05).

Hypermetropic anisometropia as well as high astigmatism was thus an important factor affecting the posttreatment visual acuity.
