　全層角膜移植術後長期の上皮の形態変化をスペキュラーマイクロスコピーを用いて観察し，半定量的，定量的解析を行った。対象は全層角膜移植術後に3年以上経過観察のできた26例33眼で，平均年齢は53.4歳，平均観察期間は5年7か月である。角膜中央部の上皮を撮影し，上皮細胞の形態を角膜上皮スコアを用いて半定量的に評価し，上皮解析システムを用いて平均露出細胞面積，その変動係数，扁平率を定量的に解析した。長期間観察例では，紡錘形細胞，大きい細胞，配列の乱れなどのスコアが正常者よりも有意に増加していた。平均細胞面積は887±290μm²，変動係数0.36±0.06，扁平率0.42±0.14で，正常者の値よりも有意に増加していた。

We observed the corneal epithelium in 33 eyes after penetrating keratoplasty. We used specular microscopy to photograph the epithelium in thecentral cornea and then scored the morphological features. The scores in the 33 eyes during 3 years after keratoplasty were significantly higher than in controls, regarding mean cell area, coefficient of variation and the cell ellipticity.
