水疱性角膜症における全層角膜移植術の予後と角膜内皮細胞の解析

Specular microscopic observation and penetrating keratoplasty for bullous keratopathy

　水疱性角膜症に対して全層角膜移植術を施行した22例23眼につき，視力予後，移植片透明率，拒絶反応の有無およびスペキュラーマイクロスコピーによる角膜内皮細胞の解析を行った。

　術後2年で70％が術前と比して2段階以上の視力向上を得ており，視力予後は良好と考えられた。移植片透明率は術後1年で91％，2年で77％，3年で67％，4年で50％であった。術後合併症のない例での角膜内皮細胞の減少率は，術後1年で17％，2年で33％，3年で66％であった。術後は拒絶反応や緑内障を高率に発生し，これらの管理が予後を大きく左右すると考えられた。

We reviewed 22 eyes of bullous keratopathy treated by penetrating keratoplasty. The grafts remained transparent in 91% of eyes during the first year, 77% in the second and 67% in the third. In eyes without allograft rejection or postoperative glaucoma, the endothelial cell loss averaged 17%during the first year, 33% in the second and 66% in the third. Allograft rejection occurred in 32% of the eyes, resulting in 81% endothelial cell loss at 2 years after surgery. Allograft rejection and glau-coma seemed to be preventable by appropriate prophylactic measures.
