角膜化学傷に対する輪部移植術の予後影響因子

Factors affecting the outcome of limbal transplantation for chemical burn

　重症の角膜化学傷に対し輪部移植術を行った17眼について，手術予後と術前の涙液機能，年齢，罹病期間，術式との関連を調べた。最終観察時に17眼中11眼（65％）で角膜の透明治癒が得られ，うち5眼が1回の手術で終了した。1回の手術で透明治癒が得られた症例は，涙液クリアランス，tearfunction index （TFl）が有意に高値であった（p＜0.05）。術後遷延性角膜上皮欠損を生じた症例は，TFIが有意に低値であった（p＜0.01）。自己輪部移植術を行った症例は，ドナー角膜から輪部移植術を行った症例より予後が良好であった（p＜0.05）。年齢，罹病期間と，予後との間に関連はなかった。輪部移植術の術式，術前の涙液検査は予後を推測するうえで重要と思われた。

We reviewed 17 eyes which underwent limbal transplantation for severe chemical corneal burn. Clear cornea was obtained in 11 eyes (65% ) , including 5 eyes which received a single procedure only. These 5 eyes showed a significantly high value of tear function index (p ＜ 0.05) . The tear function index was significantly low in cases which developed persistent epithelial defect (p ＜ 0.01) . Eyes treated by autograft limbal transplantation at the first session resulted in more favorable outcome than those treated by allograft limbal transplantation (p ＜ 0.05) . The outcome of surgery was not related either with the age of patient or the duration after injury. The dynamics of tear fluid was the most important prognostic factor for the surgical outcome.
