　14歳男性の左眼角膜輪部デルモイドに対して施行した表層角膜移植術後の角膜形状変化を形状解析装置（PKA）を用いて検討した。術式は，強角膜の一部を含むデルモイドを切除し，移植片を10-0ナイロン糸で12針端々縫合した。術後6か月までの視力と角膜乱視，角膜の形状変化をケラトメーターとPKAを用いて測定した。術直後の角膜形状は移植片を含む経線の方向に強くsteep化し，＋15Dの乱視を生じ，裸眼視力は低下した。その後強主経線方向の角膜は急激にflat化し，術後1か月目には角膜曲率は平均化し，角膜乱視はほぼ消失した。表層角膜移植術直後に生じた角膜乱視は，術後早期に回復し，その後の角膜乱視に影響しなかった。

A 14-years-old boy presented with inferotempor-al limbal dermoid cyst in his left eye as manifesta-tion of Goldenhar syndrome. The visual acuity was1.2 without correction. Measurement with photo-keratoanalyzer showed marked peripheral flatten-ing along the axis of the dermoid. We excised thecyst followed by lamellar keratoplasty.The dayafter surgery, we observed marked peripheral stee-pening along the axis of the graft. Keratometryreading showed 15 D. Uncorrected visual acuitywas 0.2. One month after surgery, uncorrectedvisual acuity improved to 0.7. Corneal mor-phometry showed less distortion, with keratometricreading of 2.5 D. Six months after surgery, thecorneal shape approximated the preoperative sta-tus with uncorrected visual acuity of 1.2.
