水晶体嚢内摘出に続くBinkhorst 4−loop lens挿入，連続383例の成績—第3報 合併症 その1 眼内レンズ脱臼と硝子体脱出眼への眼内レンズ挿入に関する成績

Results of Binkhorst 4-loop lens implantation following intracapsular cataract extraction in consecutive 383 eyes.:Ⅲ Complications. Part 1 Results in intraocular lens (IOL) dislocations and IOL implantation into the eyes with vitreous loss

　水晶体嚢内摘出に続くBinkhorst 4—loop lens挿入，連続383眼の合併症のうち，眼内レンズ（以下IOLと略）脱臼と硝子体脱出眼（以下硝脱と略）へのIOL挿入の二つに関する成績を報告した。結論は次の通りである。

　IOL脱臼28例（7.3％）あった。

　（1）個所：垂直無縫合挿入の場合，上脚は下脚の3倍脱れやすかった。硝子体内脱臼は3例あったがIOL摘出または再挿入は円滑に行えた。

　（2）治療点眼整復12例，手術整復16例。

　（3）予後全例の最終平均視力0.91であった。

硝脱例

　全47例の最終平均視力は0.97で硝脱がIOL挿入の禁忌とならないことが示された。ただし，IOL挿入時の硝脱例は視力および続発合併症の点で，水晶体娩出時硝脱例にやや劣ることが示された。

Complications occurred in 144 eyes (37.6%) of383 cases of consecutive intraocular lens implanta-tion surgery. This report evaluates the 28 eyes with IOL dislocation and 47 eyes of IOL with prolapsed vitreous.

In IOL dislocation, the upper haptics dislocated 3 times more often than the lower hap tics. Reposi-tioning of the dislocated IOL was done by eyedrops in 12 eyes and by additional surgery in 16 eyes.Dislocation of the IOL into the vitreous occurred in 3 eyes and could be successfully managed. The final visual acuity averaged 0.7. In general, the prognosis of the 28 eyes with located lens was good.
