Results of the Binkhorst 2 loop lens implantation and evaluation of lens fixation

　1979年10月〜1981年6月の間に，Binkhorst 2loop lensを挿入した46人48眼について統計的観察と，レンズ固定の分析を行った。臨床上問題となった合併症は脱臼の2眼4.2％で観血的に整復し，縮瞳剤の点眼を続行している。レンズが確実に固定されているもの44眼91.7％，レンズと嚢とが癒着していないと思えるもの，4眼8.3％で，この後者4眼と，少なくとも術後数日間は，全例縮瞳剤を与えて，虹彩支持に頼らねばならないのが，このレンズの最も神経を使うところであり欠点といえる。主に嚢内摘出後挿入する，iris medallion lensと比較するとレンズ固定はより確実であり角膜内皮損傷もより少ない。ループと嚢との癒着は，残留した前嚢が後嚢と触れる位置の水晶体上皮細胞の増殖によるものと思われた。現時点で使用に耐えうる人工水晶体の一つと思われるが，わずかであれ，なお脱臼が起りうるので，Shearing型のものが主流になっていくと考えられる。

We performed intraocular lens implantation sur-gery with the use of Binkhorst iridocapsular lens in 48 eyes during the past 21-month period. Luxation of the lens occurred in 2 eyes (4%) and could be corrected surgically. The implanted lens was fixed with the lens capsules in 44 eyes (92%) and was not fixated in 4 eyes. The non-fixation was recog-nized by the lens movement with the pupil reac-tion when illuminated under slitlamp observation. Topical miotic treatment had to be continued in these four eyes.

Topical miotics therapy had to be performed in all case for at least 3 days after surgery.
