水晶体嚢内摘出に続くBinkhorst 4−loop lens挿入，連続383例の成績—第1報 成績全般，特に術後視力を中心として

Results of Binkhorst 4-loop lens implantation following intracapsular cataract extraction in consecutive 383 eyes: Ⅰ. Outcome fo postoperative visual acuity

　1976年4月28日より1979年11月29日までの3年7ヵ月間に本院で行われたIOL挿入術のうら，水晶体嚢内摘出術に続くBinkhorst 4−loop lens（＋19.5d）挿入を受けた第1例からの連続309人383眼に関する成績を報告した。観察期間は最長3年10ヵ月，平均1年2週であった。年齢は40歳から89歳にわたり平均68歳であり，60歳以上が80.2％を占めた。同期間に行われた水晶体摘出術のうち38％がIOL挿入を受けた。術前視力は0.1以下が82.8％を占めた。要点は次の通りである。

　（1）術後視力：全症例の平均は1週日で0.5，1ヵ月目で0.8，最高視力1.0であり，最後の33例では，各々0.63,0.99,1.08であった。

　（2）最高視力に達する期間：平均は3ヵ月であった。

　（3）術後矯正度：正視が最も多く32.1％を占め，±2.0d以内のものは84.8％であった。全例の平均は−0.71dであった。

　（4）術後矯正度の変化：不変およびマイナス増加型が各々30％，次いでマイナス減少型が21％であった。

We performed implantation of Binkhorst 4-loop lens (+19.5 diopters) following intracapsular cataract extraction in 383 eyes (309 cases) during the past 43 months. The ages of the patients ranged from 40 to 89 years and averaged 68 years. They were followed up for a period from 2 weeks to 44 months (average : 12 months). The outcome of postoperative visual acuity in these consecutive 383 eyes was evaluated.

The postoperative visual acuity of all the eyes averaged 0.5 after one week and 0.8 one month after surgery. The best visual acuity of each case averaged 1.0.
