A 57-year-old male underwent intracapsular lens extraction and implantation of Binkhorst 4-loop intraocular lens in the left eye. The lower peripheral cornea developed diffuse edema 23 months after surgery. The visual acuity was 0.7. The corneal edema gradually developed to bullous keratopathy. A penetrating keratoplasty was performed 3 years and 6 months after the initial lens implantation. The result was good and the graft remained clear.

The corneal tissue removed during keratoplasty was subjected to histopathological studies. The en-dothelial cells were completely absent in the mark-edly edematous area.
