眼内レンズ挿入眼の予後とその合併症・遠隔成績

Long-term results of lens implant surgery with reference to post-operative complications

　1973年より1979年の間に眼内レンズを113眼に挿入しうち84眼について4年以上の術後経過を観察しえた。これら眼内レンズ挿入眼の長期経過後の視機能予後とその合併症について検討した。

（1）矯正視力0.5以上は69眼82％であった。（2）合併症のうち最も多かったのは後発白内障で，術後2年以上して混濁の強まるものが多かった。（3）虹彩支持レンズの脱臼は虹彩に縫着することによりその数は激減した。（4）高眼圧（21mmHg以上）を7眼に認めた。（5）高眼圧の症例中隅角にScheie III度以上の色素沈着を5眼に認め，これらはpigment dispersionとの関連が推察された。

　術後晩期に後発白内障，続発性緑内障を認める症例があり，眼内レンズ挿入はそのタイプと適応を慎重に決定して行い，術後経過良好でも十分な経過観察が必要である。

We evaluated the long-term visual prognosis and post-operative complications following intraocular lens implantation in 84 eyes. The post-operative follow-up period ranged from 4 to 10 years.

1. Final visual acuity of 20/40 or better was achieved in 82% of the eyes. 2. Development of after-cataract was the commonest complication and was observed in 11 eyes (13%). The onset of after-cataract was generally late and the vision deteriorat-ed to less than 20/40 after 2 years or more following surgery. 3.
