妊娠により増悪したと思われる鞍結節髄膜腫の1例

Exacerbation of tuberculum sellae meningioma during pregnancy

　妊娠中に視力障害，視野異常が出現し，開頭術により鞍結節部髄膜腫が確認できた本邦では初めての症例を報告し，その症状出現の機序について考察した。症例は前回の妊娠中に右視力障害が出現したが，陣痛誘発の分娩後視力は回復した。今回の妊娠も妊娠後期に右視力障害，耳側半盲が出現し遂には右視力＝0となり，左視力障害，耳側半盲も出現したが，分娩後左視力は回復した。開頭腫瘍摘出術を施行したが右視力の改善はなく，左視野の求心性狭窄を残した。この発症機序としては第1に妊娠による脳下垂体の生理的肥大，第二に原因は不明であるが妊娠による髄膜腫の増大が考えられ，これら二つがそれぞれ単独で作用したのではなく，両者相圧の作用により発症したと推論した。

A 35-year-old female developed ocular signs of meningioma of the tuberculum sellae during her third pregnancy. The visual acuity and fundus findings were normal at 3 months of pregnancy. At 6 months of pregnancy, she noted visual impa-irment in the right eye. The visual acuity was 0.2 in the right eye which also showed optic atrophy and temporal hemianopsia. The visual acuity deteriorated to zero at 8 months of pregnancy.Visual acuity in the left eye decreased to 0.5 associated with temporal hemianopsia. The visual acuity recovered to 1.2 after premature delivery two weeks later.
