硝子体手術術前検査としての超音波検査とその意義

Value of ultrasonography as a pre-vitrectomy examination

　35眼の術前超音波診断と，それに対する硝子体手術，術中術後診断を比較した。その結果次の結論をえた。

　（1）網膜挙上と診断された群には全例網膜剥離または層間断裂が見られたのに対して硝子体網膜癒着と診断された21眼のうち2眼で網膜剥離が見られ，全体としての診断的中率は94.2％であった。

　（2）網膜剥離と網膜層間断裂の超音波検査による鑑別は困難であった。

　（3）硝子体膜形成のない例や硝子体網膜癒着のない例の手術は硝子体網膜癒着のある例に比して容易であった。

　以上，硝子体手術において術前の超音波検査は極めて有用であることを確認した。

Combined B- and A-scan ultrasonography was performed in a consecutive series of 35 eyes with opaque vitreous prior to pars plana vitrectomy. These 35 eyes were classified into 4 groups accor-ding to ultrasonographic findings: simple vitreous opacity (3 eyes), vitreous membrane with out vit-reoretinal adhesion (4), vitreous membrane with vitreoretinal adhesion (21) and retinal elevation (7). These ultrasonographic evaluations were com-pared with the diagnosis during and after surgery. Overall accuracy in the diagnosis of retinalelevation was 94.2%,.
