一卵性双生児にみられた稀有なる黄斑部形成異常について

Unusual Macular Dysplasia in Monozygous Twins

緒　言

　本疾患は両眼性小眼球と＋10D以上の高度遠視に伴う特異な眼底異常として1970年，植村ら^1）により「乳頭黄斑間網膜襞」として6例がはじめて発表された。以後1975年Boynton^2）らが"Bilateral microphthalmos without microc-ornea associated with unusual papillomacu-lar retinal folds and high hyperopia"として1例報告あるのみである。

　本症の特徴は，1）両眼性の小眼球，2）＋11.0D〜＋16.0Dの高度遠視，3）黄斑部から乳頭に向つて走る網膜襞，4）矯正視力は0.1〜0.4でそれ以上には改善しない，5）眼位は正位で眼振はない，6）身体的・知能的発育異常はなく全身検査所見は正常，の諸点である。

A pair of monozygous male twins, now aged 20, presented bilateral microphthalmos and high hyperopia of +15 diopters with the best cor-rected vision of less than 0.4. While the cornea was of normal size, the ocular axial length measured about 16mm.

The most characteristic finding was a band-shaped retinal elevation from the macula ex-tending toward the disc analogous to the 'pa-pillomacular retinal fold' as described by Ue-mura and by Boynton. Fluorescein angiography revealed the absence of avascular area in the fovea.
