The Ocular Manifestation of Euthyroid Graves' Disease

緒言

　バセドー病の際に現われる眼所見は，バセドー病の経過観察上，見逃すことのできない因子として重視されているが^1）2）3），この眼所見は臨床上混乱を招く1因ともなつている。それは，バセドー病の治療をうけ，甲状線機能の亢進状態が正常化された後に，眼所見が出現してくる場合があるためである。甲状腺機能の異常によつて，眼所見の出現してくる機序が未だに明らかにされていないことにもよるが，古くから，バセドー病において，甲状腺機能状態と眼所見に相関があるような誤つた見解^4）が示されたことにもよる。著者らの調査^2）では，甲状腺の機能状態とは全く無関係に現われていた。このことから前報で，甲状腺臨床における眼の異常をdysthyroid ophthalmopathyと定義づけ，臨床的に有用な眼所見の解析を行なつた。今回は，バセドー病にみられる眼所見が現われているにもかかわらず，甲状腺機能状態が全く正常な症例に対して，眼科的な検索を加えた結果興味ある知見をえたので報告する。

The sign of lid retraction was observed in 23 (88%) out of 26 cases of euthyroid Graves' disease. The majority of these cases belonged to Type 1 of dysthyroid ophthalmopathy. It is open to question whether the euthyroid Gra-ves' disease represents the early sign or mild form of dysthyroid ophthalmopathy or whetherit has to be classified as its atypical manifes-tation.

The sign of lid retraction was also observed in 18 out of 19 cases of small goiter. The finding indicated that lid retraction is a prima-ry and important manifestation of thyroid dise-ases almost comparable to exophthalmos.
