眼窩リンパ腫の臨床的病理組織学的検討

Clinicopathological Study of Orbital Lymphoma

緒言

　眼科領域に出現するリンパ腫（lymphoma）は，Reese^1），　Forrest^3）らによると眼窩，涙腺，球結膜に多いと報告され，とりわけ眼窩にはリンパ様組織の欠如していることなどから，眼窩リンパ腫の発生病理について古くから多くの議論がなされている。また臨床的に全身の系統的なリンパ節腫大が認められ，眼窩リンパ腫が全身的な悪性リンパ腫（malignant lymphoma）の部分症状として発現してくる場合の診断は比較的容易であるが，孤立して出現してくる場合の診断はかなり困難である。さらに病理組織学的にも明らかに悪性所見を示すものから良性リンパ腫（benign lymphoma）と考えられるものまで存在する上，眼窩炎性偽腫瘍（orbital infla—mmatory pseudotumor）との鑑別などの面からもその確定診断は決して容易ではない。

　1964年より1973年までの10年間に当教室において6例の眼窩リンパ腫（細網肉腫3例，良性リンパ腫3例）を経験したので臨床的，病理組織学的に若干の考察を加え報告する。

Clinicopathological observations are presented on 6 cases of orbital lymphoma consisting of 3 cases each of reticulum cell sarcoma and benign lymphoma.

The lymphocytic tumors showed a tendency to extend anteriorly and to be well demarcated.

Two out of 3 cases of histologically confir-med reticulum cell sarcoma of the orbit later developed generalized lymphoadenopathy and died 2 and 12 months respectively after surge-ry. The third case is still being followed up 6 months after surgery.

Three cases of benign lymphoma are still alive 5 years postoperatively.
