緑内障眼におけるマ盲点の分析

Analysis of the Marione blind spot in glaucomatous eyes using a CP bracket (OCTOPUS)

　原発緑内障のBjerrurn領域における視野異常は，視神経線維束の障害として捉えられているが，その線維束の走行における視野異常，ことにマ盲点周囲の異常は興味がもたれるところである。今回，マ盲点を中心に2°間隔，66測定点を配置したプログラムを川い，CPブラケット（OCTOPUS）により，正常および緑内障眼のマ盲点の分析を行った。

　対象は高眼圧54眼および早期緑内障196眼計250眼で，正常眼53眼を対照とした。視野の程度により群（マ盲点部分的露出，マ盲点露出（I−2沈下），孤立暗点，鼻側階段および鼻側狭窄）に分別し，マ盲点の大きさおよびその周囲を，上・下・その他の3部に分け，その感度分布を調べた。

　正常マ盲点のdBポイント数（ポ数）を示すと，0 dBポ数は3.4±1.47,0〜9 dBポ数6.3±2.51,10〜19dBポ数は2.3±1.54であった。

　高眼圧54眼では，予想に反して，マ盲点上部にのみ，196の危険率で感度低下の拡大がみられた。近い将来，緑内障へ移行する例が含まれており，早期緑内障と高眼圧の鑑別が，あらためて難しい問題であることがうかがわれた。

　正常GPの緑内障眼では，マ盲点上部に有意の感度低下がみられた。ことにPOAGでは，マ盲点自体の拡大が有意に検出された。

Visual field defects in Bjerrum area in primary glaucoma are regarded as nerve fiber bundle defects. Th, distribution of sensivity around the Mariotte blind spot is an interesting subject.

We analyzed the area of the blind spot in glau-comatous eyes with a (IP bracket (OCTOPUS), with a new program at every 2' interval and with66 test ponits centering around the blind spot.

The material comprised 54 eyes with ocular hypertension and 196 glaucomatous eyes, includ-ing 98 eyes with primary open angle glaucoma, 60 with primary chronic angle closure glaucoma, 20 with developmental glaucoma and 18 with low tension glaucoma.
