先天緑内障晩発型（Developmental Glaucoma）の隅角所見に関する2,3の問題

Gonioscopic Appearance and Congenital Glaucoma Late Onset

I．緒言

　developmantal glaucoma （DG）の病因が，隅角部の発生異常によることは周知の事実である^1）〜3）。このDGには，早発型（いわゆる先天緑内障，水眼症）と，晩発型（いわゆる若年緑内障）の大部分が含まれる。ここで若年緑内障の大部分と述べたのは，若年緑内障にはDG晩発型のほかに単性緑内障の早発型が含まれるであろうからである^4）。

　さて，DG早発型はその特異な臨床像と数多くの研究が相まつて，その診断も容易になつている^5）6）。しかるに，晩発型については単性緑内障早発型とともに，若年緑内障に含まれていたために，その臨床像を追究した報告も少なく，系統的な研究もあまり多くない。若年者の眼球を摘出して，組織学的にこの種の緑内障を検索する機会が少なかつたことにもよるが，臨床的にもこの種の緑内障の診断に当たつて隅角所見の基準が明確でなかつたことにもよる。このために，DG晩発型を単性緑内障と誤つて報告されている場合が多い。ことに，単性緑内障の遺伝家系，または水眼症と単性緑内障が同一家系に現われた遺伝例などの報告の大部分は，DG晩発型ではなかつたかと著者らは考えている。

This paper is attempt to clarify the gonios-copic appearance in 7 cases with congenital glaucoma late onset in one family.

The gonioscopic appearance varies widely in extent and intensity regardless of the same clinical picture in these cases, however always consists of the remnants of mesodermal tissue such as pigment, process, band, arcade-like and membraneous structure, and the basic pattern is essentially same in every cases we have observed. There is no direct correlation bet-ween the angle appearance and the clinical picture, for these remnants of mesodermal tissue are also seen in non-glaucomatous eyes.
