いわゆるOphthalmic Graves' diseaseについて

Ophthalmic Graves' Disease

緒　言

　Gravcs病（Basedow病）は1786年Parryにより動悸を伴つた甲状腺腫，1835年Gravesにより甲状腺腫，頻脈，眼球突出を伴う女性の甲状腺疾患として，また1840年Basedow^1）により，Exoph-thamus durch Hypertrophie des Zellgenebes inder Augenhöleという表題で，甲状腺腫，著明な頻脈，身心の不安，あるいは下痢などを伴つた3例が報告されて以来，数多くの報告があり，一般的には甲状腺機能亢進症のうちで，びまん性甲状腺腫に基づくものを指していて，定型的な場合は眼球突出を伴うものである。

　しかし，甲状腺機能正常（euthyroid）にもかかわらず，眼の異常所見の認められるものもあり，1945年RundleとWilson^2）により，甲状腺腫と甲状腺機能亢進を伴わずGravcs病の眼症状を呈する症例が報告されて以来，いくつかの報告がなされている。

Three cases with ophthalmic Graves' disease are described. Two of them showed hyperthy-roidism one year after the first examination. Two cases in which CTscan was done had typical find-ings of hyperthyroidism, and the one of them developed hyperthyroidism one year later, while the other case remains euthyroid during the one-year follow-up period.

Some of the cases with ophthalmic Graves' disease often develop into typical Graves' disease and may be regarded as the previous stage ofGraves' disease. On the other hand, some cases remain to be euthyroid for a long time.
