キモトリプシンによる白内障全摘出術に関する臨床的研究（第4報）—若年者における手術成績について

Intracapsular Cataract Extraction with Enzymatic Zonulolysis:Report IV. Surgical Outcome in Juvenile Subjects

緒言

　若年者はチン氏帯が強靱であるため水晶体全摘出術は困難とされていたが，Barraquer,J．^1）によるα—chymotrypsin （以後chと略）を用いたチン氏帯の酵素離断を併用することにより，全摘出術の適応年齢をさげることが可能となつた。

　chによる高齢者に対する手術成績の報告^2）〜4）にひきつづき，今回は前報におけるものより比較的高濃度のch使用のもとに行なつた若年者にたいする白内障全摘出術の成績をまとめ，ここに報告する。なお手術成績については早期の成績とともに遠隔成績についても検討する要があるが，本稿では術中所見および比較的早期である術後1カ月までの成績について検討することとした。

Intracapsular cataract extraction was per-formed with the use of alpha-chymotrypsin in 50 cataractous eyes of juvenile subjects and the results were compared with those in senile subjects over the age of 50 years. The ages of patients in the present series ranged from 17 to 39 years (Table 1 in text).

The lens extraction could be performed with relative ease either with the use of 10,000x (150hu/ml) or 7, 500x (200hu/ml) solution of alpha-chymotrypsin applied for 1 to 2 minutes.
