キモトリプシンによる白内障全摘出術に関する臨床的研究（第1報）—老人性白内障における手術成績について

Clinical Study of Enzymatic Zonulolysis

緒言

　α—chymotrypsin （以後chと略）を用いたチン氏帯の酵素離断による白内障全摘出術は，Barraquer,J．により創始され，1958年に発表され，著者の一人田中もこの酵素離断に関して幾つかの基礎的並びに臨床的報告をしてきた^1）〜6）。

　当教室に於て本法を常用し始めてから，ほぼ10年以上を経るに至り，その間，chの使用濃度，手術方法，或いは後療法に多少の変遷をみたので，現在著者らが標準化して行なつている方法によつてch使用例（ch例と略）とchを用いず機械的断帯のみによる手術例（非使用例と略）の臨床成績を改めて比較検討することにした。尚，術後成績については，比較的早期の成績と共に遠隔成績をも検討することが必要であるが，本稿では比較的早期である術後1カ月までの成績について検討し遠隔成績については別に報告することにする。

Alpha-chymotrypsin has routinely been used as zonulolytic agent in our series of intracap-sular cataract extraction for the past 12 years in our hospital.

A control study was codnucted in 100 cases each with or witout the enzymatic zonulolysis. Only nondiabetic and uncomplicated senile cata-racts have been included in the present series.

As standard surgical procedure, four 8-0 silk sclerocorneal sutures were placed beneath the conjunctival flap where they were left to remain. 15,000-time solution (100HU/cc) of chymotrypsin was applied for 1 to 2 minutes. The age of the patients in the present series ranged from 50 to 92.
