急速に進行する緑内障性視野障害により早期発見された下垂体腫瘍

A case of pituitary adenoma detected after rapid progression of glaucomatous visual field defect

要約　目的：片眼の緑内障性視神経障害と急速に進行する視野障害から下垂体腫瘍が発見された1症例の報告。症例：53歳女性に人間ドックで左眼の視神経乳頭異常が発見され，受診した。所見：矯正視力は右1.0，左1.2で，眼圧は右21mmHg，左23mmHgであった。左眼に緑内障性視神経障害と，これに一致する視野障害があり，原発開放隅角緑内障と診断した。ラタノプロスト点眼を開始した。2年後に視野障害が急速に進行した。頭蓋内検索で下垂体腫瘍が発見され，摘出手術が行われ成長ホルモン産生腫瘍と診断された。摘出後も左眼の弓状暗点は持続した。結論：本症例では，下垂体腫瘍による進行性の視野障害が，緑内障性の視神経と視野障害に併発したと解釈される。

Abstract.　Purpose：To report a case of pituitary adenoma detected after rapid progression of unilateral glaucomatous visual field defect. Case：A 53-year-old female was referred to us for abnormal findings in the optic disc in her left eye detected during routine health-check program. Findings：Corrected visual acuity was 1.0 right and 1.2 left. Intraocular pressure was 21 mmHg right and 23 mmHg left. Her left eye showed findings compatible with glaucomatous optic disc and corresponding visual field defect. She started receiving topical latanoprost after diagnosis of primary open-angle glaucoma. Visual field showed rapid progression 2 years later. Systemic studies led to the detection of pituitary tumor. The tumor was diagnosed with hormone-producing adenoma after surgery. Visual field defect in the left eye persisted after surgical removal of pituitary adenoma. Conclusion：This case illustrates that presence of pituitary tumor may modify and accelerate glaucomatous visual field defect.
