Abstract Purpose: To report a case of posttraumatic hypotonic maculopathy who received transscleral suturing of ciliary body guided by anterior segment optical coherence tomography(AS-OCT).

Case: A 60-year-old male was referred to us for luxated intraocular lens(IOL). He has suffered penetrating injury in the right eye 23 years before. He received IOL implantation 9 years later. The IOL luxated 5 years later after blunt trauma. He received IOL implantation 5 months before. He had visual acuity of 1.2 with intraocular pressure(IOP)of 9 mmHg.

Findings and Clinical Course: Visual acuity was 0.5 right with IOP of 5 mmHg. The right eye showed findings of hypotensive maculopathy. Gonioscopy showed cyclodialysis from 2 to 4:30 o'clock in the right eye. AS-OCT showed the presence of cyclodialysis in a wider range than was observed by conventional gonioscopy. The right eye received trasscleral suturing of ciliary body. Cyclodialysis and hypotony maculopathy disappeared after surgery with visual acuity of 1.0 and IOP of 10 mmHg.

Conclusion: AS-OCT was useful in the evaluation of traumativc cyclodialysis before and after surgery.
