Purpose:To report the outcome of transconjunctival vitreous surgery using a 25-guage needle. Cases and Method:A total of 46 eyes were operated by this technique during the past 26months. They were a part of 789 eyes that received vitreous surgery during the same period. We selected eyes that do not need manipulation of peripheral vitreous and those with proliferative membrane along or inner to the vascular arcades. The series comprised diabetic retinopathy 17 eyes,epiretinal membrane 13 eyes,and macular hole 6 eyes among others. The series contained 28 phakic and 18pseudophakic eyes. Results:Best postoperative visual acuity improved in 59%,remained unchanged in 37%,and deteriorated in 4%. Final visual acuity improved in 50%,remained unchanged in 35%,and deteriorated in 15%. Complications included bleb formation 13 eyes,ocular hypotony 6 eyes,positive Seidel phenomenon 2 eyes. Conclusion:Transconjunctival vitreous surgery using a 25-guage needle allows intraocular manipulation similar to conventional vitreous surgery. Maculopathies may be one of indications for this surgical modality.
