Abstract. Purpose:To report a case of bilateral cytomegalovirus(CMV)uveitis treated with 25-gauge pars plana vitrectomy and silicone oil tamponade. Case:A 62-year-old male presented with blurring in both eyes of few days' duration. Both eyes showed iritis and elevated intraocular pressure(IOP). Treatment was followed by remission in both eyes. He was diagnosed with adult T-cell leukemia 3 weeks later. Chemotherapy was followed by repeated episodes of CMV pneumonia. Iritis recurred in the left eye 4 months after his initial visit. Retinitis progressed to bullous retinal detachment. Remission resulted after 25-gauge pars plana vitrectomy with silicone oil tamponade. Biopsy of vitreous showed CMV. Iritis and retinitis occurred in the right eye another 6 months later. Cure was obtained after similar surgery as the left eye. Both eyes developed optic atrophy with eventual blindness. The patient died of systemic complications 16 months after his initial visit. Conclusion:25-gauge vitrectomy was effective for CMV retinitis and retinal detachment.
