ヌンチャク型シリコーンチューブ挿入術における鼻内視鏡の重要性

Importance of paranasal endoscopy during intubation of nunchaku-style silicone tube

目的：ヌンチャク型シリコーンチューブ（N-ST）を挿入する際の鼻内視鏡検査の評価。対象と方法：最近15か月間にN-ST挿入術を行った涙道閉塞または狭窄がある48例50側を対象とした。鼻内視鏡で挿入前に鼻涙管の開口部の形態を確認し，下鼻道を観察しながら挿入した。39側ではN-ST抜去後3か月以上の成績を検討した。結果：鼻内視鏡検査によると鼻涙管の開口部は膜性鼻涙管が長い型が66％であり，非直視下でN-STを挿入すると誤った出口からN-STが出る危険性が高いと推定された。涙液通過障害は39側中35側（90％）で改善した。結論：N-STを挿入する際に鼻内視鏡を使うことで鼻涙管の開口部の形態が確認でき，手術成績を向上することができる。

Purpose：To evaluate paranasal endoscopy during intubation of nunchaku-style silicone tube（N-ST）for impaired nasolacrimal passage. Cases and Method：A total of 50 sides of 48 cases received N-ST in the past 15months. The inferior aperture of nasolacrimal duct was examined using a paranasal endoscope. N-ST was inserted under direct endoscopic observation. Surgical outcome was evaluated in 39 sides after follow-up of 3months or over. Results：Endoscopic observation showed that 66％ of cases have long type of membraneous nasolacrimal duct,suggesting the risk of faulty insertion without direct endoscopic observation. Lacrimal passage improved in 90％ of cases after surgery. Conclusion：Adjunctive use of paranasal endoscope allows correct diagnosis and intubation of N-ST.
