Abstract 文献概要

目的：前房または隅角にルベオーシスがある増殖糖尿病網膜症に対し，初回手術として硝子体手術を行った成績の報告。対象と方法：過去5年間に硝子体手術を行った39眼を対象とした。男性11例14眼，女性19例25眼であり，年齢は61.3±10.6歳であった。術前眼圧が23眼では21mmHg以下であり（正常眼圧群），16眼ではこれよりも高かった（高眼圧群）。結果：初回硝子体手術で，眼圧が無治療または点眼で21mmHg以下に維持されたのは，正常眼圧群19眼（83％），高眼圧群9眼（56％）であり，両群間に有意差はなかった（p＞0.14）。無治療で眼圧が21mmHg以下に維持され，かつ視力の改善が得られたのは，正常眼圧群11眼（48％），高眼圧群5眼（31％）であった。結論：ルベオーシスを伴う増殖糖尿病網膜症に対する初回硝子体手術で，高率の眼圧コントロールが得られた。

Purpose：To report the outcome of primary vitreous surgery for proliferative diabetic retinopathy with rubeosis in the iris and chamber angle. Cases and Method：This retrospective study involved 39 eyes that received vitreous surgery in the past 5 years. The series comprised 14 eyes of 11 males and 25 eyes of 19 females. The ages averaged 61.3±10.6 years. Intraocular pressure（IOP）was 21 mmHg or lower in 23 eyes（normotensive group）and was higher in 16 eyes（hypertensive group）. Results：Following primary vitreous surgery,IOP was maintained below 21 mmHg without medication in 19 eyes（83％）in normotensive and in 9 eyes（56％）in hypertensive group. The difference was not significant（p＞0.14）. IOP was maintained below 21 mmHg without medication and visual acuity improved in 11 eyes（48％）in normotensive and in 5 eyes（31％）in hypertensive group. Conclusion：Primary vitreous surgery resulted in a high rate of IOP control in eyes of proliferative diabetic retinopathy with rubeosis in the iris and chamber angle.
