We reviewed 43 eyes of 37 patients who underwent vitreous surgery for proliferative diabetic retinopathy. Macular detachment was initially present in 17 eyes (40%) and absent in 26 (60%). In the latter group, 23 eyes (88%) had visual acuity of 0.1 or more and 11 eyes (42%) had visual acuity of 0.5 or more at 3 years after surgery. In the former group, 9 eyes (53%) had visual acuity of 0.1 or more and none had visual acuity of 0.5 or more at 3 years after surgery. The observed difference in visual outcome even 3 years after surgery show that vitreous surgery is indicated before development of macular detachment in proliferative diabetic retinopathy.
