Abstract. We performed deep lamellar keratoplasty in 2 eyes of 2 patients. Both were females aged 74 and 78 years each. They had had syphilitic keratitis before puberty and had received tatooing for cornreal leucoma. Both cases showed yellowish opacity with tatooing in the lower half of cornea. The upper half was clear. The endothelial cell density was 1,700 and 2,700 cells/mm3each. During deep lamellar keratoplasty,it was difficult to separate the corneal stroma from Descemet membrane. In one eye,the graft was sutured onto the opaque stroma. The eye of the second case received penetrating keratoplasty instead. The corneal specimen from this eye showed,histopathologically,indistinct border between the stroma and Descemet membrane. We advocate fresh corneal graft for deep lamellar keratoplasty in order to facilitate intraoperative change in the surgical method.
