Deep lamellar keratoplasty involving three-fourths of the cornea was performed on 7 eyes of 7 patients with keratoconus during a 7-year period. The ages averaged 25 years. The postoperative follow-up averaged 25.7 months. Intraoperative complications included perforation of Descemet's membrane 3 eyes and subgraft hemorrhage 1 eye. Postoperative complications included temporary double anterior chamber 1 eye and graft rejection 2 eyes. The corrected visual acuity averaged 0.10 before and 0.84 after surgery. Postoperative corneal endothelial cell population ranged from 1,750 to 3,000 cells/mm2. The findings show the usefulness of deep lamellar keratoplasty in juvenile patients because of lesser incidence of graft rejection and stability of corneal endothelial cells for longer period.
