要約　目的：円錐角膜に対する全層角膜移植と深層表面角膜移植の成績を比較した報告。対象と方法：過去3年間に角膜移植を行った27例27眼を対象とした。16眼には全層移植，11眼には深層表面移植を行った。視力はlogMARで評価し，6か月以上の経過を追った。結果：術後の最終矯正視力は，全層角膜移植群が有意に良好であった（p＜0.05）。角膜形状については，surface regularity index（SRI）のみが全層角膜移植群で有意に高値であり，それ以外には両群間に差はなかった。結論：術後の最終矯正視力は全層角膜移植群が深層表面角膜移植群よりも良好であったが，これ以外には差はなかった。

Abstract.　Purpose：To compare the outcome of penetrating and deep anterior lamellar keratoplasty for keratoconus. Cases and Method：This retrospective study was made on 27 eyes of 27 cases who received keratoplasty for keratoconus in the past 3 years. Penetrating keratoplasty was performed in 16 eyes and deep anterior lamellar keratoplasty in 11 eyes. Visual acuity was evaluated as logMAR. Cases were followed up for 6 months or longer. Results：Final best-corrected visual acuity was significantly better in eyes that received penetrating keratoplasty（p＜0.05）. Among topographic parameters,surface regularity index（SRI）alone was significantly higher in eyes that received penetrating keratoplasty. There was no difference regarding other indices between the two groups. Conclusion：Final best-corrected visual acuity was better after penetrating keratoplasty than deep anterior lamellar keratoplasty. Otherwise,there was no difference between the two methods.
