防腐剤非添加緑内障点眼薬の点眼瓶の汚染

Contamination of bottle containing antiglaucomatous ophthalmic solution without antiseptic additive

要約　抗緑内障薬マレイン酸チモロール（チマバック^(R)）を含む点眼瓶の汚染の可能性を検索した。21名の高眼圧症または緑内障患者に2種類の点眼薬をそれぞれ1週間使用させた。防腐剤を含まないチマバック^(R)点眼薬と，防腐剤である塩化ベンザルコニウムを含むマレイン酸チモロール点眼薬である。回収した点眼瓶口付着物の培養を行った。それぞれ21検体中，前者からの8検体，後者からの12検体に細菌が検出された。両群の検出率に有意差はなかった。分離された細菌について，塩化ベンザルコニウムを付加したときの最小発育濃度（MIC）を評価した。グラム陽性菌に対するMICは低く，陰性菌に対しては高かった。以上の結果から，チマバック^(R)点眼瓶の汚染は少なく，グラム陰性菌への塩化ベンザルコニウムの抗菌力は弱いと結論される。

Abstract.　We evaluated the risk of contamination of bottle containing timolol maleate,an antiglaucomatous agent. Twenty-one glaucoma patients were asked to use two ophthalmic solutions for one week each. One bottle contained timolol alone and the other contained timolol and benzalkonium chloride.Incubation of smears from the bottle neck showed bacteria in 8 samples from the former and in 12 samples from the latter. There was no significant difference in the rate of contamination between the two groups. We then examined the isolated organisms regarding the minimum inhibitory concentration（MIC）after addition of benzalkonium chloride. Gram-positive bacteria showed low MIC and gram-negative ones showed high MIC.The findings show that bottles containing timolol alone have a low risk of contamination after clinical use. Benzalkonium chloride has a limited antiseptic effect for gram-negative bacteria.
