Abstract. We performed cataract surgery on 90 eyes of 45 patients. Each patient received acrylic intraocular lens(IOL)in one eye and silicone IOL in the other. Their ages ranged from 39 to 90 years,average 70.1 years. Acrylic IOL was inserted through a 3.5 mm sclerocorneal incision with forceps. Silicone IOL was inserted through a 3.0 mm incision with an injector. Visual acuity on day 2 of surgery was significantly better in eyes receiving silicone IOLs. Induced astigmatism was greater in eyes receiving acrylic IOLs except one month after surgery. There were no differences between the two groups regarding intraocular pressure,refractive error from predilected value,or aftercataract.We advocate smaller incision and procedure inducing less stress for cataract surgery in elder patients.
