網膜硝子体手術のクリニカルパス

Clinical pathways for retinal and vitreous surgery

　網膜硝子体手術の医療チーム用クリニカルパスとして増殖糖尿病網膜症硝子体手術，網膜剝離硝子体手術，黄斑円孔手術，その他の硝子体手術，網膜剝離バックル手術の5種類のクリニカルパスを導入した。2003年1月までの1年間に，初回の網膜硝子体手術を行った患者は66名（69眼）で，クリニカルパスは69眼（100％）に使用されていた。クリニカルパス導入により術後在院日数の短縮が得られた。バリアンス（逸脱症例）は24眼（35％）に生じた。バリアンスは増殖糖尿病網膜症硝子体手術パスを用いたものが最も多く，バリアンスの原因で多かったのは硝子体出血の吸収遅延と術後眼圧上昇であった。

　We implemented 5 types of medical team clinical pathways for retinal and vitreous surgery. They included proliferative diabetic retinopathy,retinal detachment,macular hole,other vitreous diseases,and scleral buckling for retinal detachment. We reviewed the actual use of clinical pathways and variances during 1 year period. Retinal-vitreous surgery was performed on 69 eyes of 66 patients as the initial procedure. The hospital stay significantly shortened after the implementation of the clinical pathways. The overall incidence of variance was 35％. The variance occurred most often in proliferative diabetic retinopathy. Prolonged vitreous opacity after surgery and postoperative intraocular pressure elevation were the major causes of variances.
