糖尿病網膜症に対する硝子体手術成績と硝子体トリプル手術の適応

Outcome of vitreous surgery for diabetic retinopathy and indications for simultaneous vitrectomy,phacoemulsification and intraocular lens implantation

要約　糖尿病網膜症47眼に対して硝子体手術を行った。8眼は当初から眼内レンズ（IOL）挿入眼であり，これ以外の39眼では水晶体切除を併用し，うち29眼にはIOL同時挿入によるトリプル手術を行った。これら29眼には術前にルベオーシスがなく，手術終了時に広範な網膜剝離がなかった。術後に最終視力が2段階以上改善したものは，術前IOL挿入眼100％，トリプル手術眼86.2％，IOL非挿入眼50％であり，3群間で術後合併症に差がなかった。上記29眼で採用した硝子体トリプル手術の適応基準は妥当と考えられた。

Abstract.　We performed vitreous surgery on 47 eyes of diabetic retinopathy. Eight eyes were pseudophakic prior to surgery. The other 39 eyes received simultaneous phacoemuslification-aspiration（PEA）. Out of the 39 eyes,29 eyes received additional intraocular lens（IOL）implantation,or triple procedure. These 29 eyes had no rubeosis and no extensive retinal detachment at the end of surgery. Visual acuity improved by two lines or more in 100％ of pseudophakic group,86.2％of eyes receiving triple procedure,and 50％ of eyes receiving vitrectomy and PEA only. There was no difference in postoperative complications among the three groups. The findings show the soundness of indications for triple procedure as described above.
