多剤耐性のCorynebacterium speciesが検出された角膜潰瘍の1例

A case of corneal ulcer due to Corynebacterium speciesc resistant to multiple antibiotics

　65歳，男性が左眼の視力低下と疼痛で受診した。6年前に左眼が角膜炎と診断され，以後再発を繰り返し，抗菌薬やステロイド薬の治療を受けていた。18日前に水疱性角膜症が発症していた。20年前に糖尿病が発見され加療中であったが，コントロールは不良であった。矯正視力は右1.0，左光覚弁であった。右眼に異常所見はなかった。左眼には角膜中央に円形の潰瘍があり，潰瘍底はデスメ膜瘤になっていた。眼脂と結膜充血が顕著であった。眼脂の培養でCorynebacterium sp.が検出された。薬剤感受性検査で，ペニシリン，セフェム，テトラサイクリン，グリコペプチド，クロラムフェニコール系に感受性があり，ニューキノロン，アミノグリコシド，マクロライド，リンコマイシン，ホスホマイシン系に耐性を示した。感受性のあるテトラサイクリン系の眼軟膏とセフェム系の点滴で眼脂が減少し，3か月後に角膜潰瘍は瘢痕治癒した。コントロール不良の糖尿病，ステロイド薬と抗菌薬の長期点眼，水疱性角膜症による局所のバリア機能低下が感染の誘因になったと推定された。

A 65-year-old male presented with pain in his left eye. He had had recurrent keratitis in the past 6 years and had been treated by antibiotics and corticosteroid. Bullous keratopathy was detected 18 days before. He had had poorly controlled diabetes mellitus since 20 years before. His visual acuity was 1.0 right and light perception left. His left cornea showed an ulcer with descemetocele in its center. His left eye showed hyperemia and profuse discharge.Corynebacterium sp. was isolated from the discharge. It was sensitive to penicillin,cefems,tetracycline and chloramphenicol. It was resistant to quinolones,aminoglycosides,macrolides,lincomycin and fosfomycin. Topical tetracycline and intravenous cefems induced prompt improvement of corneal ulcer and cicatricial healing 3months later. The corneal ulcer seems to have occurred due to poorly controlled diabetes mellitus,prolonged use of antibiotics and corticosteroid,and compromised corneal surface by bullous keratopathy.
