　硝子体手術を行った裂孔原性網膜剝離60眼を回顧的に検索した。全例が50歳以上で，平均59歳であった。黄斑円孔網膜剝離とC2以上の増殖硝子体網膜症は除外した。全例に後部硝子体剝離があった。初回手術で55眼（92％）が復位し，最終的に58眼（97％）が復位した。復位しなかった2眼は，観察期間終了時にシリコーンオイルが未抜去であった。平均視力は，黄斑非剝離眼では術前0.86，術後0.81であり，黄斑剝離眼では術前0.01，術後0.37であった。以上，後部硝子体剝離に伴う裂孔原性網膜剝離に対する硝子体手術の成績はおおむね良好であった。増殖硝子体網膜症が発症して再手術を行った症例と術前の黄斑剝離期間が長い症例では，復位が得られても術後視力が不良であった。

　We reviewed 60 eyes of rhegmatogenous retinal detachment treated by vitreous surgery. All the cases were aged 50 years or over. Their ages averaged 59 years. Cases of macular hole retinal detachment or proliferative vitreoretinopathy were excluded. All the eyes had posterior vitreous detachment. The retina became reattached in 55 eyes（92％）after initial surgery and in 58 eyes（97％）after repeated surgeries. The visual acuity averaged,in eyes without macular detachment,0.86 before and 0.81 after surgery. It averaged,in eyes with macular detachment,0.01 before and 0.37 after surgery. Cases of postoperative proliferative vitreoretinopathy or prolonged macular detachment resulted in poor final visual acuity. Primary vitreous surgery without these two factors was effective for rhegmatogenous retinal detachment.
