We performed vitreous surgery for proliferative diabetic retinopathy in both eyes of a 61-year-old female. Both eyes showed thick and taut fibrovascular membranes in the posterior fundus. Segmentation appeared to be too difficult. An illumating probe was inserted through the fourth port by an assistant followed by delamination of proliferative membranes using en bloc excision technique. Perfluorocarbon liquid was used to flatten the retina. Panretinal photocoagulation was performed after removal of proliferative membranes.The retina became reattached and the visual acuity improved from hand motion to 0.1 in either eye 13 months after surgery. The present case illustrates the usefulness of en bloc delamination and removal of thick and taut proliferative membranes using a bimanual technique.
